Use this form to submit general inquiries and resource requests related to campuswide initiatives led by the Office of the Senior Vice Chancellor for Diversity, Equity and Inclusion. Please note that form submissions are not anonymous, and your name and email address will be visible to office administrators. If our office cannot address your questions or concerns, your message will be routed to the appropriate campus unit.
University policy requires our office to relay reports of discrimination, harassment, sexual misconduct, intimate partner violence and stalking to the Office of Institutional Equity and Compliance (OIEC). If you’d like to submit a report to OIEC, please do so through the OIEC website. OIEC offers the option of submitting a report anonymously.
