Instructor Provisioning

If you have five (5) or more instructors to provision at once, please email an excel file with the appropriate information to academic scheduling.

Instructor Information
If you have three or more requests to submit at once, please attach the completed Instructor Provisioning excel form for multiple submissions:
One file only.
100 MB limit.
Allowed types: txt, rtf, pdf, doc, docx, odt, ppt, pptx, odp, xls, xlsx, ods.

To make adjustments to user or instructor name(s):
Update Preferred Name
Initiate Legal Name Change
