The campus Medical Services team is hosting a Q&A session about the Daily Health Questionnaire on Thursday, July 9 at noon. Please register to attend if you would like to learn more about the use and purpose of the questionnaire. You can ask questions and gain a better understanding about why this is being used as a safety measure for the campus.

You’re receiving this invitation because you have either completed the form at some point since it was made available or because you are the supervisor of an employee that has used the form.

Here are questions we’ve been hearing since the form launched:

• What happens with the information once the form is completed?
• What am I supposed to do as a supervisor when an employee completes the form?
• Is the information submitted secure?
• What will Medical Services do if I report having COVID-19 symptoms?

We’ll address these questions and more during the Q&A via Zoom.
